This has been the demo level also.
From the start of the level, turn right and walk into the room behind the metal gate. Use the double shotgun to work through the neanderthals appearing behind you.
Then, fire the ball into the goal to get a 100% extra health.
Before crossing the fist deep gap via the ventilators, turn around on the first ventilator. Jump into the gap above the hall you came from.
The extra life comes with the 10.000 points, not with the secret. After collecting the money bags, turn around the next corner to kill some bonsai Orc Grunts.
Before entering the hover fighter, visit the tree to its left to get this secret. Bring down all pepperonis with a grenade to get some health.
